About the role:

A Community Connections Officer, will focus on building and strengthening connections within Plymouth community.
Community Connection Officer will focus on supporting vulnerable individuals and groups. Community Connection Officer responsibilities include providing advice, support, and resources related to housing, community issues, and social inclusion. As a Community Connection Officer you will be working to prevent homelessness and promote wellbeing.

Key Responsibilities:

• Preventing Homelessness:

  Assessing needs, providing support, and connecting individuals and families with relevant resources to maintain stable housing.
• Community Problem Solving:

  Addressing neighborhood issues, promoting community resilience, and resolving conflicts.
• Providing Advice and Support:

  Offering guidance on housing options, community services, and other relevant issues.
• Managing Accommodation:

  Efficiently managing temporary accommodation and ensuring appropriate support for individuals in these settings.
• Client Interaction:

  Conducting interviews, gathering information, and providing support to clients facing housing or community challenges.
• Partnership Working:

  Collaborating with local agencies, support teams, and other relevant organizations to deliver comprehensive support.
• Promoting Social Connection:

  Facilitating opportunities for community participation, volunteering, and social interaction.
• Case Management:

  Recording case information, monitoring progress, and ensuring effective support is provided.
• Maintaining Records:

  Ensuring sensitive and personal information is handled appropriately and in compliance with regulations.
